Enrollment Information

New students to the Mammoth Unified School District will need to provide the following:

• Birth certificate

• Vaccination records - more info about vaccination requirements below

• Two proofs of residency in the district (utility bill, lease agreement, building contract, mortgage, etc.)

• Student's last report card

Enrollment Packet

Please print and completely fill out the attached Enrollment Packet.  Bring the completed packet along with the items noted above to the front office to get the registration process started.
